Obtain a Fingerprint Card

The FD-258 card is the standard fingerprinting card within the US. If you are located in the US, before requesting a card, please check the facility at which you will be fingerprinted to determine whether they will provide an appropriate card or require you to bring your own. 

If needed, call or email the CBCP and request a fingerprint card by mail. Be sure to include the following information:

• First and Last Name — If leaving a voicemail, be sure to spell your last name.
• Date of birth
• Board to which you have applied
• Mailing address where the card should be sent — In most cases, it is helpful to spell street and city names.
