​​New state webpage separates fraud facts from fiction​
Accurate, fact-based information is critical in the fight against fraud. The Minnesota Department of Human Services launched a new fact check webpage to correct misleading information and outright false claims about Medicaid fraud in Minnesota. 

Minnesota advances work on Medicaid anti-fraud systems
Work to update Minnesota’s Medicaid claims processing systems using advanced analytics has cleared its first hurdle. The state expects to have a complete pre-payment review process in place by the end of the year.

Notice of data breach

On January 16, 2026, the Minnesota Department of Human Services mailed clients notification of a data breach involving its MnCHOICES system. The system is used by counties, Tribal Nations, and managed care organizations to support their assessment and planning work for Minnesotans needing long-term services and supports. 

DHS strongly encourages anyone with information about suspected Medicaid fraud to contact DHS by calling 651-431-2650 (Toll-free: 1-800-657-3750) or by submitting a report online at https://mn.gov/dhs/general-public/office-of-inspector-general/report-fraud.


IRS form 1095-B is available upon request

DHS will mail the form only when requested by Medical Assistance and MinnesotaCare enrollees. We will not automatically send the form out. Follow the instructions on this webpage to request your form 1095-B.
