Q.  What types of forms might they fill out/information will they need to release to you?

 “The Student Consultant will assist the student in completing standard intake forms (e.g., name, contact information, functional limitations, agreed upon accommodations, etc.).  Additional forms may be required, depending on factors such as functional limitations and accommodations to be provided.”
