Primary
care clinics (PCCs) can change cost levels each year, which will affect how
much you pay out-of-pocket for health care services. Click here to view the health
plan schedule of benefits. Cost level 1 clinics have the lowest out-of-pocket
costs for members, while cost level 4 clinics have the highest costs.
To
identify primary care clinics in your area: Start by entering a location such as an address,
city and state, ZIP code, or building name in the Location Search field below.
If your first search doesn’t return any results, try adding the state or county
name of the area you are looking for. Optional filters can also be used to
narrow search results by distance, cost level, or health plan administrator
(BlueCross BlueShield, HealthPartners). You can find primary care
clinics using location search terms like:
- City and State
(St. Paul, MN)
- Buildings or
Landmarks (Minnesota State Capitol)
- County (Ramsey
County, MN)
- ZIP Code (55155)
