DOC provides a person-centered approach to transition services by offering a wide array of topics designated to help people prepare for release and connect with services and supports. These opportunities range from self-help resources through targeted services for those with greatest need. Each DOC facility maintains a Transitions Center with dedicated Transition Coordinators. Access to these centers is available to all incarcerated people.
- Healthcare Insurance is critical to help people access treatment for mental and physical issues as well as substance use disorders. Incarcerated people are highly encouraged to apply for health coverage prior to release. Staff assist them in completing the necessary forms and submitting these to the appropriate entities. If people are leaving prison without a permanent address, we may also offer support in applying for public assistance benefits through Bridging Benefits, a Department of Human Services partnership.
- Housing plans are very important. Before release, the DOC provides educational topics and resources to help people identify places to live to create stability for their future. Topics like tenant rights, creative housing search, financial literacy, and more. For more information, email housingsupports.doc@state.mn.us
- Identification is important, especially for those releasing from prison. ID is critical for job seeking, applying for benefits, opening bank accounts, and more. As a priority service, DOC staff work with the incarcerated to apply for three critical ID documents, as needed - birth certificate, duplicate social security card and Minnesota State Identification Card or Driver's License renewal. The DOC and the Department of Public Safety (DPS) partner to provide photo ID equipment at all DOC facilities, allowing people to secure state photo ID cards or driver license renewals close to their release date. DOC staff also provide the opportunity to take the web based Class D Knowledge Exam at each DOC facility.
