Providers access their previously submitted tool through the web-based 2015 Quality Improvement Tool. After selecting the "Return to an existing form" option, enter the NPI/UMPI number and email address used previous submission.
If you answered all the required questions and successfully submitted the Quality Improvement Tool, you fulfilled the quality improvement requirement. The full rate increase is maintained. You do not need to wait to receive an approval notice from DHS to begin implementing your project.
Yes. Providers who have previously submitted a plan through The 2014 Quality Improvement Tool must submit either an update of continued work for their ongoing project or submit a new quality improvement plan. Providers submitting an update will be asked about their project status, changes to the project, and successes or learnings gained.
