Eldercare Development Partnership Grants

The Minnesota Department of Human Services, through its Aging and Adult Division (State), is seeking Proposals from qualified responders to form and lead Eldercare Development Partnerships in accordance with Minnesota State Statutes, section 256B.0917, and other applicable State and Federal laws and regulations. 

Purpose of the Eldercare Development Partnership Grants

  • Develop a local long-term services and supports strategy consistent with state goals and objectives
  • Make strategic changes in the long-term services and supports system for older adults 
  • Provide statewide capacity for local service development and technical assistance 
  • Identify and use local skills, knowledge, and relationships, and build on these assets

  • Grant term 12 months (July 1, 2022 - June 30, 2023) 
  • Match requirement; 25 percent match
