There is increasing call for health professionals to provide more holistic care to their patients. This includes discussing healthy lifestyles choices and advance care planning decisions. These broader discussions also include long-term care planning, including such topics as talking to family members about care preferences, and end-of-life discussions and decisions. Most people see their primary care physician or other health care provider as a resource in this decision-making process.
Own Your Future will develop outreach strategies with health care professionals to reach their colleagues with this message and identify resources they can share with their patients on the issue of creating a long-term care plan.
