Provider alert survey for critical COVID-19 pressures

COVID-19 Provider Alert Survey

To help better explain the critical pressures your agency may experience during these challenging times, DHS is conducting a survey of:

  • Personal Care Assistance (PCA) Agencies
  • Psychiatric Residential Treatment Facilities (PRTF)
  • Residential Treatment Services (IRTS)
  • Substance Use Disorder (SUD) residential providers
  • Adult Foster Care
  • Supported Living Service providers

Information collected will be shared with key stakeholders and policy-makers to shape their understanding about pressures including: 

  • COVID cases or suspected cases due to symptoms among staff and people served
  • Personal protective equipment (PPE) and other monitoring equipment
  • Staffing and service capacity.

The survey is voluntary. Survey responses will inform emergency response strategies that address the needs of older adults, people with disabilities and people who are homeless.

Complete the survey

Complete an initial survey to report any critical pressures your facility or program currently experiences. When pressures change, please submit a follow-up report. We want to know when new pressures arise or when existing ones are addressed.
