Your Shot to Summer Rewards

 

Minnesota’s Your Shot to Summer program has ended and we are no longer accepting submissions. 

 

If you have previously requested a reward, we are processing your request and will be in contact shortly. Learn more about how you will receive your reward here. For questions about your reward submission, call the MDH COVID-19 Public Hotline at (833) 431-2053 Monday through Friday 9 a.m. - 7 p.m. and Saturday 10 a.m. - 6 p.m.

 

If you have not yet been vaccinated, you can find a location near you using the Vaccine Locator Map. 

 

If you received a first dose of the Pfizer or Moderna vaccines, please remember to receive your second dose at your scheduled time. If you need to schedule a second dose appointment, visit mn.gov/vaccine to find a provider near you.
