OT Licensing

In the State of Minnesota, you must have a current Minnesota Occupational Therapy license before practicing or using protected titles. Any licensing or processing fees incurred are your responsibility and are nonrefundable. The Board reserves the right to reject or cancel any outdated applications submitted. Applications will not be considered complete until all required documents are received by the Board office.
There are several methods to use when applying for a new full license. Please review the following pages to determine the method that fits your circumstance. All versions require completion of an online application, submission of nonrefundable fees and provision of supporting documents based upon license type. All supporting documents must be sent directly from the originating organization to the Board office.
Criminal Background Check:
Effective January 1, 2018, MN Statute requires all applicants applying for a new full license to complete a Criminal Background Check. Detail instructions will be sent by email from the CBC office after submission of application and all relevant fees. Immediate action is advised to avoid potential delays in the application process.
