Fee Amounts

The fees imposed in this part shall be paid by cash, personal check, bank draft, cashier's check, or money order made payable to the Board of Executives for Long-Term Services and Supports. All fees are nonrefundable. The fee amounts may be set by the board with the approval of the Department of Management and Budget up to the limits provided in this part depending upon the total amount required to sustain board operations under Minnesota Statutes, section 16A.1285, subdivision 2. Information about fees in effect at any time is available from the board office. The maximum amounts of fees are outlined in State Rules 144A

Nursing Home Administrator Application Fees

Initial application for licensure, $150.

A prospective applicant may have their education and experience reviewed in order to obtain waivers prior to submitting a full license application for $100. If an application is submitted within one year of the educational review, the $100 fee will be applied to the fee for the licensure application.

Minnesota state examination fee is $100

The fee for initial licensure varies based on the month the license is issued. Initial license if issued between July 1 and December 31 is $225, and $100 if issued between January 1 and June 30. This is specific to NHA applicants.
Criminal Background Check

Minnesota law now requires that all applicants and license reinstatement must complete a fingerprint-based criminal background check Minn. Stat. (§ 214.075). The Minnesota Health Licensing Boards have cooperatively established the Criminal Background Check Program to help you efficiently complete this mandatory background check. Fingerprints are crosschecked with databases of the Bureau of Criminal Apprehension and Federal Bureau of Investigation. Applications for licensure and license reinstatement will have to pay the application fee and the criminal background check fee( $33.25).

After your application with proper fees is received, the Criminal Background Check Program will send you a packet containing additional information and directions. You are responsible for having your fingerprints taken promptly and for completing all required paperwork so as to not delay finalizing your license application. Previously taken fingerprints cannot be used. Some agencies charge a fee for fingerprinting services.

NOTE: Background checks are valid for 12 months from the date they are completed.
Temporary Acting Administrator Permit

An acting administrator permit is limited to 6 months and cannot be renewed per Minnesota Rule 6400.6700 . In extenuating circumstance facility's may requests a waiver to the board for unanticipated and special circumstances and the board may issue a second permit to serve a facility as an acting administrator for up to six months from the expiration of the original acting administrator permit when the board finds the second permit to be in the best interests of the public. In no event, shall the board issue successive permits for a total duration of longer than one year. All temporary/acting permit administrator applicants must pass the BELTSS' Minnesota State Exam for NHA to qualify for their permit

The Acting Permit application fee is $250.00. The Minnesota State Exam for NHA is $100.00.

The below fees are the fees due at the the time of application. Their are other cost associated with meeting the requirements for licensure such as exam fees.

ALD Education (31)

Total fee due at the time of application: $233.25 This payment will cover the completion of your application, your CBC (valid for 12 months) and for your first license.

ALD Endorsement (32)

Total fee due at the time of application: $233.25 This payment will cover the completion of your application, your CBC (valid for 12 months) and for your first license.

ALD MN NHA Approved (33)

Total fee due at the time of application: $233.25 This payment will cover the completion of your application, your CBC (valid for 12 months) and for your first license.

Assisted Living Director-in-Residence (ALD- In- Residency(IR) (34))

Total fee due at the time of application: $458.25. This payment will cover the completion of your application, your CBC (valid for 12 months), the ALD state exam (one attempt), one residency permit and your first license.

Assisted Living Director-in-Residence Shared Director Assignments.

The initial director permit paid for in the fees for your Assisted Living Director-in-Residence (ALD- In- Residency(IR) (34)) application. For each additional license a $125.00 fee is required for the board to issue each additional wall certificate/license. This fee may be paid by check or money order made payable to BELTSS.
