Requesting a License / Registration Verification
State-to-State Online Verification for Physician and Physician Assistant (PA) Licenses
The Minnesota Board of Medical Practice contracts with VeriDoc Inc. to use its website to expedite the verification process of physician and physician assistant licenses to participating licensing boards in other states. The board transfers licensure information to VeriDoc, Inc. daily. Go to http://www.veridoc.org to have a verification letter sent to another participating state board. You may select verification of your physician assistant license or physician license (inc. temporary permit and/or residency permit) or just your temporary/residency permit/telemedicine registration to another participating state.
If you need an endorsement verification containing the Minnesota exam scores for your medical license, send a written request with $40.00 check or money order to the above address. Endorsements cannot be done through VeriDoc.

You can use the following search tools to find a practitioner and lookup licensure, registration, training, and disciplinary actions, or to verify a licensure/registration issued by the Board, including Acupuncturists, Athletic Trainers, Traditional Midwives, Naturopathic Doctors, Physician Assistants, Professional Firms, Physicians and Surgeons, Respiratory Therapists and Telemedicine.
The license information in the search results has been designed and implemented to meet primary source verification requirements of the Joint Commission accredited hospitals and the National Committee for Quality Assurance (NCQA) certified managed care organizations, and it can be used as the primary source verification. The above search tools won’t display the temporary license, temporary registration and residency permit information. Please email the Board, email@example.com , if you didn’t find a provider.
