Online Collaborative Agreement

Licensed hygienists in the State of Minnesota are encouraged to register their collaborative agreements with Minnesota Board of Dentistry. The Board desires to track all collaborative agreements since the inception of the statute in 2001. If a dental hygienist has more than one collaborative agreement, each one needs to register separately. Each collaborative agreement can only have one hygienist, one dentist, and effective in August 2017; dental assistants may also be included in the collaborative agreement. There is no fee and the process only takes a couple of minutes. If a hygienist was in a past collaborative agreement that is now dissolved, the agreement can be registered with the Board, and then indicated as dissolved.

To log in and register you will need:

  • Your name
  • Your social security number
  • Your license number
  • The license number of the dentist involved in the collaborative agreement
  • The facility name, address, phone and contact name

Dissolved Agreement:  You must notify the Board if you dissolve your registration or have dissolved a past agreement. To do this, please click on the link listed below. You will need:

  • Your name
  • Your social security number
  • Your license number

Note: If you have the same collaborative agreement with more than one dentist, a dissolution per dentist must be entered in the registry.
