Online Collaborative Agreement

Minnesota Licensed Hygienists need to register their collaborative agreements with Minnesota Board of Dentistry per Minn. Stat. 150A.10, Subd 1a(c)(5). The Board wants to track all collaborative agreements since inception of the statute in 2001. If a dental hygienist has more than one collaborative agreement, each one needs to be registered separately. Each collaborative agreement can only have one hygienist and one dentist. There is no fee. It will take only a couple of minutes. If a Hygienist has been in a collaborative agreement and it has since been dissolved, the Board would like to track that as well. A past agreement must be registered with the Board, and then indicated as dissolved.

Background:  The Allied Education Committee of the MN Board of Dentistry has been studying Collaborative Agreement/Practice. As the numbers of collaborative agreement/practicing hygienists have increased, it has become apparent that a registry for tracking who and where the collaborative practices are located is necessary. State legislators and professional organizations are looking to the MN Board of Dentistry for answers regarding numbers and locations of collaborative practices.

To log in and register you will need:

  • Your name
  • Your social security number
  • Your license number
  • The license number of the dentist involved in the collaborative agreement
  • The facility name, address, phone and contact name

Dissolved Agreement:  You must notify the Board if you dissolve your registration or have dissolved a past agreement. To do this, please click on the link listed below. You will need:

  • Your name
  • Your social security number
  • Your license number

Note: If you have the same collaborative agreement with more than one dentist, a dissolution per dentist must be entered in the registry.
