As of January 2, 2017 the Minnesota Board of Dentistry will no longer be offering affidavit/verification letters. Instead, state dental boards wishing to access Minnesota dental professional licensee information can access this information from our Online Licensee Verification Section.
If the licensee has been subject to public discipline, the detail of the orders may be located on our website under Public Actions. If the disciplinary order cannot be located there or you need information on a public corrective action, the state board should email dental.board@state.mn.us with a request, licensee name and license number.
