Address and Name Change

Licensees must be licensed under their full legal name. Per Minnesota Rule 2500.1105, licensees must notify the MN Board of Chiropractic Examiners (MBCE) within 30 days of any name or address change. Failure to notify the Board may create difficulties receiving renewal and other important notices; and is considered a violation of the Practice Act.

Address change: You may update your address, phone and email at any time by logging into your online account: Address Change Online. 

We must have a current email address that goes directly to the licensee (not the clinic) for official Board communications, such as renewal reminders. Without a current email address, you may not get these notices. The board does not share or post your email address. 

Name changes require official legal documentation (i.e. Original or Certified copy of the marriage certificate, divorce decree, or court order authorizing the name change). Due to the requirement of Original or Certified documentation, a name change must be processed by paper/mail. Fill out and print the "Name Change" form and mail to the Board office along with the official legal documentation: Name Change Request form

Your original/certified documents will be returned to you by U.S. Mail.
