List Requests

A list of Doctors of Chiropractic licensed in Minnesota may be purchased by mailing the DC List Request Form along with the the appropriate fee (see form for details).  The Minnesota Board of Chiropractic Examiners does NOT give out email addresses, websites, or fax numbers of any licensee of the Board.
