Completing the FD-258 Fingerprint Card

NOTICE: The building at 335 Randolph Avenue is closed to the public until April 22, 2022.

Fingerprints must be submitted to the Criminal Background Check Program (CBCP) on a fingerprint card. CBCP staff continue to telework part time. The office is staffed daily; however, the office may not be staffed during all business hours. We recommend sending fingerprints and forms by USPS or courier (e.g., UPS, FedEx) rather than bringing them to the office in person.

Fingerprints must be submitted to the CBCP on the official FBI-issued FD-258 card.

What is the FD-258 Card?

The FD-258 card is the official fingerprint card produced and approved by the FBI. The current version of this card contains the full Privacy Act Statement, as required by the FBI. A card that is equivalent to an FD-258 card may be used if all the following apply:

  • Fields for required demographic data must be on the card
  • Fingerprint layout must be in the same order and in similar position on the card
  • A PAS Certification Form is now a CBC requirement. This form replaces the requirement for the Privacy Act Statement on the back of the card.

How can I get a fingerprint card if I need one?

The FD-258 card is the standard fingerprinting card within the US; if you are located in the US, before requesting a card, please check the facility at which you will be fingerprinted to determine whether they will provide an appropriate card or require you to bring your own. If needed, call or email the CBCP and request a fingerprint card by mail. Be sure to include the following information:

  • First and Last Name — If leaving a voicemail, be sure to spell your last name.
  • Board to which you have applied
  • Mailing address where the card should be sent — In some cases, it is helpful to spell street and city names.
