A community clinic that is state-certified to offer comprehensive, person-centered and family-centered, trauma-informed, evidence-based behavioral health and substance use disorder services in an integrated, one-stop-shop model. Provides a variety of services to children and adults with complex mental health and substance use disorders so that they do not have to piece together care across multiple providers, increasing the likelihood that care is received before a person enters into a crisis. Core services provide: * Crisis services available 24/7 * Care coordination with other providers and systems * Screening, assessment, diagnosis and risk assessment * Outpatient mental health and substance use disorder services * Mental Health Targeted case management for children and adults * Outpatient primary care screening and monitoring * Community based mental health care for veterans * Peer, family, and recovery support services * Person-centered and family-centered treatment planning * Psychiatric rehabilitation services for adults and children * Outpatient withdrawal management services

A Consumer Directed Community Supports (CDCS) Support Planner helps people to write and use the person-centered Community Support Plan that is needed for CDCS. CDCS allows participants in waiver programs or Alternative Care to manage their own services and supports. These personalized supports help people to live at home and avoid or delay out-of-home placement. CDCS Support Planner services could include: * Providing information about CDCS, provider options and community resources * Helping to write a person-centered CDCS Community Support Plan * Helping to personalize a CDCS Community Support Plan based on the person's preferences and assessed needs * Helping the person to put their plan into action, and to monitor, evaluate and revise their plan * Helping to recruit, screen, hire, train, schedule and monitor workers The person and/or family caregiver decides how much help they need from the CDCS Support Planner.

Mental Health Case Managers coordinate services, design treatment plans and plan for crises to serve people of all ages with mental illness. Plans for mental health services are customized for each person's needs and designed to help people live with as much independence within the community as possible. Services may assist the person in gaining access to a variety of services including mental health, medical, social, educational, housing, vocational and recreational.

Semi-Independent Living Services (SILS) help people to reach their goals and lead self-directed lives in the community. Services include training and assistance in independent living skills for adults with developmental disabilities or related conditions. Services provided may include training and assistance in: * First aid and getting help in an emergency * Learning and exercising rights and responsibilities of community living * Meal planning and preparation * Obtaining and maintaining a home * Personal appearance and hygiene * Self-administration of medication * Shopping * Social, recreation and transportation skills, including appropriate social behavior * Using the phone and other utilities * Managing money * Engaging in activities that make it possible for adults with developmental disabilities or related conditions to live in the community SILS staff may provide supported employment in community work environment settings for a recipient who already has a job but may be experiencing some challenges in maintaining it, or is at risk of losing such employment. This requires direct contact between the SILS recipient and staff person providing Support Employment. A SILS recipient may receive a one-time housing allowance of up to $1,500 to cover costs related to damage/security deposit for housing rental, utility deposit, household furnishings and other items necessary that will enable the person to secure a residence in which to receive SILS.

Family Assets for Independence in Minnesota (FAIM) is a 30-month program that helps working Minnesotans with low incomes increase their savings, build assets and enter the financial mainstream. By matching a participant's savings as well as providing financial literacy education, asset-specific training and ongoing financial coaching, families are supported and encouraged to acquire assets and increase their financial capability and economic security, in order to leave poverty permanently. Those enrolled in FAIM receive a program match at a rate of $3 to every dollar of earned income deposited into their Individual Development Account (IDA) within 2 years, up to a maximum of $2,000 per household. Account holders may be eligible for a matched payout and their savings of up to $8,000 toward their chosen asset upon successful completion of program requirements. Savings and matches may be used for: First-time home purchase, post-secondary education, emergency savings, 529 college savings, purchase of a personal vehicle or a small business start-up or expansion. Participants are required to complete a minimum enrollment period, financial education classes/coaching, as well as asset-specific training and requirements before requesting payout toward obtaining their chosen asset. FAIM payment is made to a third-party vendor, not the participant.

Programs may include crisis intervention and domestic violence education and prevention.
