A support worker is someone trained to help people that need support to take care of their day-to-day activities. Support workers help people live and work within their own communities. These support workers help with daily living needs because of disease, a disabling condition, or complications related to aging. Activities they can help with include: * Daily living activities * Health related activities * Behavior observation and monitoring Community First Services and Supports (CFSS) provider agencies support people eligible for CFSS who select the CFSS agency model by employing their workers and performing all employment tasks such as: recruiting, hiring, training, supervising and evaluating support workers (they do not support individuals in the CFSS budget model). The individual receiving care is still responsible for directing their own care and has a say in selecting their support worker, setting worker's schedules and can participate in the training and supervising of their support worker.

Services are directed at maintaining or improving a person's ability to self-care. This includes health, social and nutritional services. Services are directed toward the achievement of specific outcomes identified after assessment in a service plan. This service may be provided either in-person or remotely. Semi-Independent Living Services (SILS) help people to reach their goals and lead self-directed lives in the community. Services include training and assistance in independent living skills for adults with developmental disabilities or related conditions. Services provided may include training and assistance in: * First aid and getting help in an emergency * Learning and exercising rights and responsibilities of community living * Meal planning and preparation * Obtaining and maintaining a home * Personal appearance and hygiene * Self-administration of medication * Shopping * Social, recreation and transportation skills, including appropriate social behavior * Using the phone and other utilities * Managing money * Engaging in activities that make it possible for adults with developmental disabilities or related conditions to live in the community SILS staff may provide supported employment in community work environment settings for a recipient who already has a job but may be experiencing some challenges in maintaining it, or is at risk of losing such employment. This requires direct contact between the SILS recipient and staff person providing Support Employment. A SILS recipient may receive a one-time housing allowance of up to $1,500 to cover costs related to damage/security deposit for housing rental, utility deposit, household furnishings and other items necessary that will enable the person to secure a residence in which to receive SILS.

Crisis lines provide immediate support to help ensure a person's safety and help them take the next steps toward resolving a problem. Crisis intervention hotlines/helplines are available through telephone, email, chat, text or other communication methods. A Consumer Directed Community Supports (CDCS) Support Planner helps people to write and use the person-centered Community Support Plan that is needed for CDCS. CDCS allows participants in waiver programs or Alternative Care to manage their own services and supports. These personalized supports help people to live at home and avoid or delay out-of-home placement. CDCS Support Planner services could include: * Providing information about CDCS, provider options and community resources * Helping to write a person-centered CDCS Community Support Plan * Helping to personalize a CDCS Community Support Plan based on the person's preferences and assessed needs * Helping the person to put their plan into action, and to monitor, evaluate and revise their plan * Helping to recruit, screen, hire, train, schedule and monitor workers The person and/or family caregiver decides how much help they need from the CDCS Support Planner.

This program provides support to people in abusive situations and works to reduce relationship abuse, intimate partner violence and family violence. Programs may include crisis intervention and domestic violence education and prevention. Shelter offering a safe space for battered women, their families and others who have experienced domestic abuse MORE INFORMATION House of Peace is a temporary shelter for Asian women and their children who are victims of domestic violence. Note: The shelter is geared toward the Asian community but will accept women of all national origins and races.
