The adult foster care service covers the provision of protection, supervision, household services, help with living skills or training and support for safeguarding cash resources. Foster care services are individualized and based on the needs of the person, as identified in the support plan. There are two types of Adult Foster Care: * Family Adult Foster Care - In Family Adult Foster Care the license holder lives in the home and is the primary provider of care. * Corporate Adult Foster Care - In Corporate Adult Foster Care the license holder does not live in the home and is not the primary provider of care. Trained and hired staff generally provide services. In Minnesota there is not a service called "group home." When the term "group home" is used, it is usually referencing the corporate adult foster care service. A support worker is someone trained to help people that need support to take care of their day-to-day activities. Support workers help people live and work within their own communities. These support workers help with daily living needs because of disease, a disabling condition, or complications related to aging. Activities they can help with include: * Daily living activities * Health related activities * Behavior observation and monitoring Community First Services and Supports (CFSS) provider agencies support people eligible for CFSS who select the CFSS agency model by employing their workers and performing all employment tasks such as: recruiting, hiring, training, supervising and evaluating support workers (they do not support individuals in the CFSS budget model). The individual receiving care is still responsible for directing their own care and has a say in selecting their support worker, setting worker's schedules and can participate in the training and supervising of their support worker.

A Personal Care Assistant (PCA) is a trained worker who helps people that need support to take care of their day-to-day activities and who are on a state program (Medical Assistance, waiver programs, or Alternative Care). Personal assistance services help individuals live and work within their own communities. PCAs help people who need support to take care of daily living needs because of disease, a disabling condition, or complications related to aging. Personal care assistance services must be deemed medically necessary as determined by an assessment. The assessment must find that the person needs help to complete at least one of eight activities of daily living or exhibit Level 1 behavior. The four categories of services that a person can receive assistance in are: * Activities of Daily Living: eating, toileting, grooming, dressing, bathing, transferring (such as getting in and out of bed), mobility, and positioning * Health-related functions, which, under state law, can be delegated or assigned by a licensed health care professional to be performed by a PCA * Instrumental Activities of Daily Living: meal planning and preparation, managing finances, shopping for essential items, performing essential household chores, communication by telephone and other media, and participating in the community * Redirection and intervention for behavior including observation and monitoring If you are enrolled in Medical Assistance, a waiver program, or Alternative Care, there are two possible ways that you may receive PCA services in Minnesota: * You may select an individual worker (where you are responsible for hiring, supervising and firing) * You may hire a home care agency who selects and oversees your worker If you choose to hire your own workers, you will also choose a PCA Choice provider (usually a home health agency) as a fiscal intermediary. The PCA Choice provider is responsible for billing the Department of Human Services (DHS) for PCA services and paying your PCA workers. PCA has begun transitioning to Community First Services and Supports (CFSS). Individuals currently using PCA will transition to CFSS at their next reassessment.

A licensed assisted living facility provides sleeping accommodations and assisted living services including: * Assistance with dressing, eating, oral hygiene, hair care, grooming, using the bathroom and bathing * Providing standby assistance * Providing verbal or visual reminders to the resident to take regularly scheduled medication, which includes bringing the resident previously set up medication, medication in original containers, or liquid or food to accompany the medication * Providing verbal or visual reminders to the resident to perform regularly scheduled treatments and exercises * Preparing specialized diets ordered by a licensed health professional * Services of an advanced practice registered nurse, physician assistant, registered nurse, licensed practical nurse, physical therapist, respiratory therapist, occupational therapist, speech-language pathologist, dietitian or nutritionist, or social worker *Tasks delegated to unlicensed personnel by a registered nurse or assigned by a licensed health professional within the person's scope of practice * Medication management services * Hands-on assistance with transfers and mobility * Treatment and therapies * Assisting the person with eating when they have complicated eating problems * Providing other complex or specialty health care services * Awake staff available 24/7 to respond to residents' health and safety needs The Uniform Disclosure of Assisted Living Services and Amenities (UDALSA) and the Assisted Living Bill of Rights is available to all current and perspective residents as required by MN Statute 144G. A class series that supports caregivers in taking care of yourself while caring for someone else. By taking care of your own health and well-being, you become a better caregiver. Six classes held once a week are led by experienced class leaders. Class participants are given The Caregiver Helpbook to accompany the class and provide additional caregiver resources. Focuses on improving self-care by learning a new set of skills or "tools." Topics include: * Reduce stress * Improve self-confidence * Manage time, set goals, and solve problems * Communicate in challenging situations * Make tough decisions * Locate helpful resources A national program based on the Stanford Chronic Disease Self-Management Program. Program that helps people understand and successfully complete forms, paperwork or documents that are required to receive benefits or services MORE INFORMATION * Provides assistance in completing immigration forms and supporting documents. * Will help with translating, completing and reviewing County or State eligibility forms and applications. * Assist with United States naturalization, green cards-permanent residency, employment authorization forms, family reunification, Visas, adjustment status, and National Visa Center processing forms * Will accompany a client at interviews and follow ups related to immigration applications filed by their representative.

This provider teaches eligible people about their Community First Services and Supports (CFSS) options to help them make informed choices about what works best for them, within program guidelines. People using CFSS have many options and choices. Consultation services staff help the person review those choices and develop their CFSS service delivery plan. All people using CFSS must select a consultation services provider.
