Information, guidance and support for family and other informal caregivers, typically through in-person meetings, over the phone or using online communications. MORE INFORMATION This program offers the following services to caregivers of Korean elders aged 60 or older: * Caregiver support group * Caregiver training and counseling * Caregiver education * In-home senior respite care The Special Access program provides social services to the elders by exposing them to the mainstream programs for their qualified services.

Caregiver Consultation is a service that can help caregivers develop strategies to achieve a balanced lifestyle, allowing the caregiver to provide both good care and protect their own health. It is offered throughout Minnesota by professionals who are certified Caregiver Consultants. Consultants are certified by the Minnesota Board on Aging by meeting basic standards and completing training. Caregiver Consultation involves identifying a caregiver's strengths, needs and goals, and developing a plan with the caregiver. Caregiver consultants are available to: * Unpaid family, partners, and friends who provide care to an older adult (60 years or older) * Unpaid family, partners, and friends who provide on-going care to younger persons with dementia * Persons age 55 or older who live with and care for a grandchild, other relative's child, or an adult child with a disability Caregiver Consultants may provide: * Caregiving education and skills training * Self-care and stress management suggestions * Disease-specific information to support caregiving * Guidance on managing family and care team relationships * Communication and assertiveness skills training * Support in developing informal networks of support * Family meeting facilitation The Caregiver Consultant service is part of the Minnesota Board on Aging's Live Well at Home program.

Respite provides care on a short-term basis for a person who cannot take care of themselves, and who is usually taken care of by a family member or friend. The service is designed to provide temporary coverage for the caregiver during an absence or need for relief. An ombudsman is a neutral party who investigates consumer complaints. They work with both the consumer and the organization to resolve consumer issues and improve customer service. MORE INFORMATION Provides a fair, neutral and transparent environment between state and county agencies and families of color in Minnesota Each of the following communities has an Ombudsperson: the Indian Affairs Council, the Chicano Latino Affairs Council, the Council on Black Minnesotans, and the Council on Asian-Pacific Minnesotans An Ombudsperson is an independent government official responsible for reviewing government and government-regulated agencies in an effort to ensure that their practices are fair, reasonable and appropriate. To do this, the ombudsman: * Receives complaints solely active child protection investigations and/or court cases * Reviews and investigates complaints * Makes recommendations to the agencies to resolve the complaints Types of issues Office of the Ombudspersons for Families can assist with: * Child Protection matters, child out-of-home placements and foster care issues * Unjust or discriminatory treatment by child protection services related to an active case Cannot help with: * Cannot give legal advice * Cannot investigate court decisions * Does not investigate attorneys, police, judges, or elected officials, or their decisions and actions * Does not investigate federal agencies

Program that works on behalf of individuals and/or groups to make sure that they receive the benefits and services for which they are eligible MORE INFORMATION The Center focuses on providing assistance for Chinese immigrants, refugees, from children to elderly in various social service areas, such as: * Social Security, Supplemental Security Income (SSI), and Social Security disability * Medicare, Medical Assistance, MinnesotaCare * Public Housing * Child care assistance, school information, Head start, Women, Infants and Children (WIC) * Employment referral and unemployment benefits * Immigration counseling * Psycho-social counseling * Translation * Crisis intervention * Health care information and referral

Recreational activities for older adults to create social connections, learn new things and improve their general well-being. MORE INFORMATION Friendship and fellowship for elderly people of Japanese descent living in Minnesota Activities include: * Monthly meetings with food and fellowship * Yearly events * Japanese library, available to members This is a Japanese-speaking group. A number of the volunteers speak English. Accepts volunteers who want to support the welfare of Japanese elders in Minnesota.

Providers with a comprehensive home care license are licensed by the Minnesota Department of Health. These services are for people who, because of an illness, disability, or physical condition, cannot perform the tasks for themselves, or who cannot travel to receive health services. A licensed comprehensive home care agency may provide the services of licensed health professionals such as: * Nurses * Physical, speech-language, and occupational therapists * Dieticians or nutritionists * Social workers The services may include medication management; complex medical care like tube feedings and ventilator care; hands-on assistance with transfers and mobility; treatment and therapies; assisting clients with eating when the clients have complicating eating problems; and other health-related procedures. Sometimes health-related tasks are delegated to and performed by unlicensed personnel under the supervision of a registered nurse or other licensed health professionals. Home care providers with a comprehensive license also assist people with non-medical tasks, such as going to the bathroom, bathing, and transferring from one location to another. In addition, some home care providers with a state-issued comprehensive license are Medicare certified. Medicare certification indicates that the agency has met additional federal standards and inspections. Medicare certification can be important if the person getting services seeks payment from insurance or Medicare. Check under Licenses and Certifications to see if this agency is Medicare certified.

This provider is registered as a "home management provider" by the Minnesota Department of Health. Tasks allowed with a home management registration include: housekeeping, meal preparation, and shopping. Home management registration does not allow the provider to perform home health care services to clients. For example, home management services do not include bathing, dressing, or grooming. A home management registration also does not allow the provider to help clients with medication, special diet preparations or feeding assistance.

Shelter offering a safe space for battered women, their families and others who have experienced domestic abuse MORE INFORMATION House of Peace is a temporary shelter for Asian women and their children who are victims of domestic violence. Note: The shelter is geared toward the Asian community but will accept women of all national origins and races.

Crisis lines provide immediate support to help ensure a person's safety and help them take the next steps toward resolving a problem. Crisis intervention hotlines/helplines are available through telephone, email, chat, text or other communication methods. Providers with a basic home care license are licensed by the Minnesota Department of Health. These providers can assist with most activities of daily living, in a person's home. Basic home care licensed providers may be individuals or organizations and are trained to assist people who, because of an illness, disability, or physical condition, cannot perform the tasks for themselves. This type of service does not require a nurse. Staff of the licensed basic provider may: * Assist with dressing, eating, grooming, toileting and bathing; * Provide standby assistance for safety; * Provide verbal or visual reminders to take regularly scheduled medication; * Provide verbal or visual reminders for treatments and exercises; * Prepare modified diets ordered by a licensed health professional; and * Assist with laundry, housekeeping, meal preparation, shopping, or other household chores if also providing one of the other tasks listed above The workers performing the tasks may be individually licensed or unlicensed. Licensed agencies are responsible for providing additional training for their workers as required by home care laws.

A class series that supports caregivers in taking care of yourself while caring for someone else. By taking care of your own health and well-being, you become a better caregiver. Six classes held once a week are led by experienced class leaders. Class participants are given The Caregiver Helpbook to accompany the class and provide additional caregiver resources. Focuses on improving self-care by learning a new set of skills or "tools." Topics include: * Reduce stress * Improve self-confidence * Manage time, set goals, and solve problems * Communicate in challenging situations * Make tough decisions * Locate helpful resources A national program based on the Stanford Chronic Disease Self-Management Program.
