A person-centered assessment and support-planning process for people who need assistance caring for themselves due to age, disability or illness
DESCRIPTION:
The MnCHOICES (https://mn.gov/dhs/people-we-serve/people-with-disabilities/services/home-community/programs-and-services/mnchoices.jsp) process helps identify a person's support needs and the services and programs that could meet those needs. It also helps a person access those services and programs. MnCHOICES is an online application used by counties, tribal nations and managed care organizations to support their assessment and support planning work.
Certified assessors from lead agencies (counties, tribal nations and managed care organizations) use the MnCHOICES assessment within the MnCHOICES application to:
* Interview the person about their need for services and supports
* Identify potential services and program 
* Give the person a summary of what was learned in the assessment interview and next steps based on their needs  
The MnCHOICES assessment determines eligibility for publicly-funded long-term services and supports, including:
* Alternative Care (AC)

DESCRIPTION:
Adult foster care means the provision of food, lodging, protection, supervision and household services to a functionally impaired adult in a residence. Adult foster care may also include personal care, household and living skills help or training, medication help and support safeguarding cash resources.
Applicants apply for an Adult Foster Care (AFC) license through their county. An adult foster care license holder may have a maximum license capacity of five if all persons in care are age 55 or over and do not have a serious and persistent mental illness or a developmental disability. 
There are three types of licensed foster care settings that serve adults:

DESCRIPTION:
Helps pay housing-related expenses for people with low income who are 65 years or older, or are 18 years or older and have a disability that prevents them from working enough to support themselves. People can receive Housing Support from an eligible provider if they live in an eligible place, which could be a licensed facility or a person's own home.   
In some situations, Housing Support may provide additional supportive services for people who are not receiving waiver or Personal Care Assistant (PCA) services.

County office that oversees licenses for adult foster care and community residential settings. Services include housing, protection, household services, supervision and food for five or fewer functionally impaired adults.
DESCRIPTION:
Adult foster care means the provision of food, lodging, protection, supervision and household services to a functionally impaired adult in a residence. Adult foster care may also include personal care, household and living skills help or training, medication help and support safeguarding cash resources.
Applicants apply for an Adult Foster Care (AFC) license through their county. An adult foster care license holder may have a maximum license capacity of five if all persons in care are age 55 or over and do not have a serious and persistent mental illness or a developmental disability. 
There are three types of licensed foster care settings that serve adults:
* Family Adult Foster Care is where the foster care home is the primary residence of the license holder and they are the primary care giver.
* Corporate Adult Foster Care is a home where services are provided that is not the primary residence of the license holder.
* Community Residential Setting is a home where services are provided that is not the primary residence of the license holder and all individuals are funded by a disability waiver.
