Certified assessors from lead agencies (counties, tribal nations and managed care organizations) use the MnCHOICES assessment within the MnCHOICES application to: * Interview the person about their need for services and supports * Identify potential services and program * Give the person a summary of what was learned in the assessment interview and next steps based on their needs The MnCHOICES assessment determines eligibility for publicly-funded long-term services and supports, including: * Alternative Care (AC) * Brain Injury (BI) Waiver * Community Alternative Care (CAC) Waiver * Community Access for Disability Inclusion (CADI) Waiver * Developmental Disabilities (DD) Waiver * Elderly Waiver (EW) * Community First Services and Supports (CFSS) The MnCHOICES assessment is also used to help identify other programs and services to explore, including: * DD case management (Rule 185) * Housing Stabilization Services * Moving Home Minnesota * Relocation service coordination targeted case management * Essential Community Supports (ECS) * Semi-Independent Living Services (SILS) When the MnCHOICES assessment determines that a person is eligible for publicly-funded long-term services and supports, staff will provide the person information so they are able to make an informed choice about waiver services and other state programs.
