Personal Care Assistance Assessment
Available through the tribal government (as alternative to county assessment for Band members)
A Personal Care Assistant (PCA) Assessment is provided at no charge in the home of elders and people with disabilities.
An assessment by a public health nurse to determine the amount of time that can be authorized based on State and Federal guidelines. The participant then chooses an agency to provide the services, which can be from any agency in the state.

A team of a nurse, an occupational therapist and a handy worker addresses both the home environment and uses the strengths of the older adults themselves to improve safety and independence.

* The goal is to decrease difficulty with Activities of Daily Living, reduce symptoms of depression, and improve motivation
This project is funded by a Minnesota Department of Human Services Live Well at Home Grant.
