DESCRIPTION: Case managers help people to plan, access, evaluate and coordinate services from their service providers. Plans are customized to each person's own needs and preferences. MORE INFORMATION Case management services for adults 60 years of age and older and their families. Services include: * Assessment and development of a care plan * Coordination of services * Advocacy and mediation with health care providers, insurance company and home care agency * Application assistance * Russian-speaking staff is available.
