Eligibility

Adults and children residing within the 7 county metropolitan area with Intellectual/Developmental Disabilities and Related Conditions as well as individuals with a Mental Health Diagnosis receiving services through a Community Access for Disability Inclusion (CADI) Waiver.

Referrals for behavioral support can be initiated by phone at (612) 869-6811, 9:00am - 5:00pm, Monday - Friday

  • Early intervention assessments
  • Crisis support planning
  • Coordinates admissions and discharges to 11 crisis homes (44 beds total) throughout the metro area
  • Caregiver and provider training programs
