Application process
If you think your animal may be eligible for the program, email the agency with a description of your animal's medical needs.
Include a phone number where you can be reached with your email as well.
If your animal's medical needs match the program criteria, they will advise you to complete the application.
There is a wait list and eligibility requirements which will be explained during the phone call. Do not send the application before staff has had the opportunity to review your email request for assistance.

Program that supports pet owners by providing free or low-cost services, pet food or other supplies.
MORE INFORMATION
Provides financial assistance to owners of domestic animals who need veterinary care.
Funds non-basic, non-urgent care, such as cancer treatment, heart disease, chronic conditions, endocrine diseases, and eye diseases.
It does not fund basic care, urgent care or emergency care.
Funds are paid directly to the care provider for services, not the pet owner.
