If you think your animal may be eligible for the program, email the agency with a description of your animal's medical needs. Include a phone number where you can be reached with your email as well. If your animal's medical needs match the program criteria, they will advise you to complete the application. There is a wait list and eligibility requirements which will be explained during the phone call. Do not send the application before staff has had the opportunity to review your email request for assistance.
