Eligibility

If transportation is covered through insurance or a county, the eligibility is set by the payer Wheelchair and ambulatory clients welcome. Includes one vehicle with a stretcher in Mille Lacs, Sherburne, Benton and Wright counties Will pick-up from Benton, Cass, Mille Lacs, Sherburne, Wright counties and transport to anywhere in the state

Application process

Pre-approval is required through the Medicaid insurance company and documentation needed for Medicaid appointment over a 30, 60 mileage. Transportation available Monday - Saturday, flexible times if pre-scheduled. Transportation hours: 4:00am - 6:00pm.
