Case managers help people to plan, access, evaluate and coordinate services from their service providers. Plans are customized to each person's own needs and preferences. Learn more about case management costs that are covered through waivers in the Community-Based Service Manual. MORE INFORMATION Services include:
  • Information and consultation about community resources and county, state, and federal programs for seniors and individuals with disabilities
  • Resources for personal care, equipment and supplies, meals, living skills training, cleaning, meal preparation and respite services to help stay in the home
  • Needs assessment
  • A short-term service plan with the family
  • Case management for mental health or developmental disabilities
  • Help for parents struggling to access services
