Mental Health Case Managers coordinate services, design treatment plans and plan for crises to serve people of all ages with mental illness. Plans for mental health services are customized for each person's needs and designed to help people live with as much independence within the community as possible. Services may assist the person in gaining access to a variety of services including mental health, medical, social, educational, housing, vocational and recreational. MORE INFORMATION Provide case management services to people with Serious and Persistent Mental Illness (SPMI) in Hennepin, Dakota and Ramsey counties Case managers help clients gain access to services needed to meet their mental health needs, achieve recovery, and live independently in the community.
