Housing for people with low income, who are homeless or at risk of homelessness, that includes coordination of other services for residents. The coordination professional can help residents identify resources, navigate systems or help with applying to additional programs. MORE INFORMATION Provides supportive services in housing for homeless and disabled youth throughout the Twin Cities. Supportive housing is provided in an apartment building and 48 units of scattered-site housing in Hennepin County. These sites serve homeless and/or disabled young people and are designed to increase self-sufficiency and decrease the risk of future homelessness. Lindquist Apartments

Housing First
  • 30 units of scattered site supportive housing for youth ages 18 - 24 experiencing homelessness that also have a mental health and/or chemical dependency diagnosis or a physical disability
Periscope Program
  • Serves young adults between the ages of 18 - 24 in Hennepin County who have been referred by Hennepin County social workers, probation officers, other community partners, or self-referred
  • Assists participants in locating and maintaining stable housing within Hennepin County using local private landlords
Project Live Out Load
  • Rapid Rehousing program for youth who identify on the LGBTQ spectrum ages 18 - 24
Dignity House provides emergency shelter for youth ages 10 - 17 of all gender identities who have run away from home, are experiencing homelessness and/or who are involved in the child welfare and juvenile justice systems. Accepts referrals from any geographic location. The Rapid Rehousing Program
  • Helps youth ages 18 - 24 who are experiencing homelessness transition quickly from the streets and emergency shelters to stable housing
  • Housing for up to 24 months with support in building skills and increasing income to successfully maintain housing after completing the program
