Case managers help people to plan, access, evaluate and coordinate services from their service providers. Plans are customized to each person's own needs and preferences. MORE INFORMATION Provides Human Immunodeficiency Virus (HIV) medical case management to ensure its members have access to important supportive services, information and assistance. Assists with:
  • Navigating early access to and maintenance of health care
  • Assistance and advocacy in applying for social security and public aid
  • Help in understanding complex private health insurance issues
  • Locating and applying for resources such as:
  • Emergency financial aid
  • Food stamps and nutrition resources
  • Housing vouchers
  • Utilities assistance
  • Employment
  • Transportation
  • Referrals for medical, psychological, financial, legal, and other services
  • Life skills such as personal safety, budgeting
