Eligibility

All veterans enrolled in the Standard Medical Benefits Package are eligible if they meet the clinical need for the service and it is available.

Application process

Veteran eligibility determination followed by Home Based Primary Care (HBPC) staff evaluation of appropriateness for HBPC services.

Fees

Copay may be charged based on disability status and financial information.

Site Service Area

Primarily the St. Cloud area, but will serve veterans eligible for VA health care regardless of place of residence.
