Staff will complete an in-take interview over the phone to make sure that new patients qualify for services. New patients (or their guardians) will receive a packet in the mail with forms to complete and return. Once the clinic has received the paperwork, staff can schedule an appointment.

Most insurance plans are accepted. Patients enrolled in a Minnesota Health Care Program, including MinnesotaCare and medical assistance, are also eligible.
