The coordination professional can help residents identify resources, navigate systems or help with applying to additional programs. MORE INFORMATION * Permanent Supportive Housing - Includes a rent subsidy where individuals or families pay 30 percent of their income for rent. The program includes home visits and case management services based on need. * Chronic Permanent Housing - Provides weekly home visits for case management. The program also includes a housing subsidy, so a person would only pay 15 percent of their income for rent, and there is no time limit for how long they can stay on the program * Housing Support Program - Case management provided based on needs
