Outpatient health clinic offering preventative, diagnostic (exam or test) and treatment services Some clinics are certified as Health Care Homes, where primary care providers, families and patients work together to support wellness and improve health outcomes. The Minnesota Department of Health website provides a complete list of Health Care Home Clinics along with an online map of these clinics in Minnesota. MORE INFORMATION Basic health care offered at a medical clinic, to low income, homeless or uninsured populations who are not eligible for government programs. There are no walk-in services. Care includes: * Health care services * Outreach and early intervention with diabetes education, health fairs, flu immunizations, screenings * Advocacy Clinics are located in spaces donated by churches, schools, and community centers in the metropolitan area. Directions to the closest clinic will be given when an appointment is made. Staff can assist qualified patients in applying for MNsure and state programs such as MinnesotaCare and Medical Assistance (MA) to support long term health care access
