Case managers help people to plan, access, evaluate and coordinate services from their service providers. Plans are customized to each person's own needs and preferences. MORE INFORMATION Provides American Indian elders with the support they need to maintain their health and wellness Services include transportation for elders with mobility issues, access to nutritious food and other commodities, connections to community through social events, support during difficult times and assistance with applying for or accessing other community resources. The Grandparents Caring for Grandchildren program provides support and resources to grandparents who have taken on the role of primary caregiver of a child/ren not more than 18 years of age.
