Provides long-term support programs that help people with disabilities as they live in the community.
Disabilities can include:
- Developmental disabilities
- Chronic medical conditions
- Acquired or traumatic brain injuries
- Physical disabilities
- Assistive technology training and education
- Economic assistance programs - help with housing, nutrition and income
- Environmental modifications and provisions
- Personal support services - home care services, home delivered meals, personal care assistance
- Social Security Advocacy - help to apply or maintain benefits
- Health care programs - such as community dental clinics, chemical dependency services
- Trainings - providing life skills and employment opportunities
