Description

Provides long-term support programs that help people with disabilities as they live in the community. Disabilities can include: * Developmental disabilities * Chronic medical conditions * Acquired or traumatic brain injuries * Physical disabilities Provides such support services as: * Assistive technology training and education * Economic assistance programs - help with housing, nutrition and income * Environmental modifications and provisions * Personal support services - home care services, home delivered meals, personal care assistance * Social Security Advocacy - help to apply or maintain benefits * Health care programs - such as community dental clinics, chemical dependency services * Trainings - providing life skills and employment opportunities * And more Disability Hub MN is a free statewide service that helps people find options to live independently. Disability Hub MN is managed by the Department of Human Services Disability Services Division, and the service is delivered through a contract with the Metropolitan Center for Independent Living (MCIL) and the Southeastern Minnesota Center for Independent Living (SEMCIL)
