A rehabilitation agency is an agency that provides a combination of treatment and education services to restore maximum functioning, a sense of well-being, and a personally satisfying level of independence for people who have temporary or permanent disabilities. At a minimum, a rehabilitation agency must provide:

• Physical therapy or speech pathology services
• A rehabilitation program with social or vocational adjustment services

This provider is a Medicare Certified Outpatient Physical Therapy or Outpatient Speech Pathology or Outpatient Occupational Therapy provider and is subject to periodic inspection by the Minnesota Department of Health. MORE INFORMATION Offers physical therapy in addition to other medical services
