9:00am - 3:00pm, Monday - Thursday (Pacific Time)
They require all patients to complete each of the online forms in order to qualify for assistance. Paperwork must be received within the required time frame, at least 14 days prior to the departure date. Downloadable applications may be submitted online. They encourage people to call if they have questions about how to fill out the forms.

Provides help to fly patients, personnel or equipment to healthcare facilities. Flights are typically provided by volunteers. Programs can also support these services through donations or informational resources. MORE INFORMATION Provides financial assistance for airline tickets for those in need of specialized, out-of-state medical care. Flight assistance is also provided to two parents/legal guardians traveling with the child patient and one companion or caregiver for adult patients. Travel assistance is provided for qualified individuals to retrieve a service dog or attend service dog training as required by service dog organizations.
