Eligibility

Residents of Ramsey County that have a diagnosed mental illness of a serious and persistent nature. This organization provides housing referrals/resources to ex-offenders reentering the general population from a correctional facility.

8:00am - 5:00pm, Monday - Friday

Individualized, flexible support and community treatment services, provided when and where needed; designed to help individuals who have serious and persistent mental illness and intensive service needs live in the housing of their choice. Assistance with medication and daily activities; other community support services available. Case management services for those referred by Ramsey County. Assertive Community Treatment also available.
