Supervised Living Facility

DESCRIPTION: A supervised living facility provides supervision, lodging, meals, counseling and developmental habilitative or rehabilitative services to five or more persons who are developmentally disabled, chemically dependent, adult mentally ill, or physically disabled. Some types of supervised living facilities are for persons who can take care of themselves in an emergency. Other facilities are for persons unable to take care of themselves in emergencies. Proper placement in a supervised living facility will depend on the services and care needed by the person. This provider is licensed as a supervised living facility by the Department of Health and is subject to periodic health and safety inspections for regulatory compliance by the Minnesota Department of Health. Services include: * Clinically managed withdrawal management * Rule 25 access * Onsite physician visits * Comprehensive Assessment * Counseling * Nutritional Services * Chemical health education services * Alcoholics Anonymous (AA)/Narcotics Anonymous (NA) groups * Discharge planning * Referral

Fee

Accepts private pay and insurance. For clients who meet financial eligibility requirements, all services are billed to the county.

Eligibility

Men, women and adolescents over age 13; need for services must originate in the following counties: Hennepin, Anoka, Scott, Carver, Chisago, Isanti, McLeod, Mille Lacs, or Sherburne. There are no limits as to county of origin for clients who utilize private insurance or are self-pay.
