Program is for nursing home residents who have a disability, are eligible for medical assistance, and no longer need the level of medical care provided at a skilled nursing facility.

8:00am – 4:30pm, Monday - Friday
Application process
Referrals from a county case worker.
Fees
Paid for through waivers or private pay.

This program helps people who wish to move out of a licensed facility into stable community housing. The program also helps clients find the medical and social services and supports needed to live independently, such as Personal Care Attendants, who can help with essential daily activities, like bathing, dressing and feeding. Many people relocating from nursing homes have very few of the basic household items needed to start over again. The Transitional Services program can help with the transition to a new home by providing furniture and household items like bed sheets, towels, and cooking equipment, and can also arrange for delivery of these items to the new home. If you qualify for the programs these services are provided at no cost.
