Mental Health Case Managers coordinate services, design treatment plans and plan for crises to serve people of all ages with mental illness. Plans for mental health services are customized for each person's needs and designed to help people live with as much independence within the community as possible. Services may assist the person in gaining access to a variety of services including mental health, medical, social, educational, housing, vocational and recreational. MORE INFORMATION Services that help adults with Serious and Persistent Mental Illness (SPMI) get access to needed medical, social, educational, job support, and other necessary services as they relate to the recipient's mental health needs. Services include: * Assessment * Community Support plan * Referral and assisting people with persistent mental illness in getting needed mental health and other services * Coordination of services
