Adults with a recent diagnostic assessment of a serious and persistent mental illness, or children diagnosed with a severe emotional disturbance.

8:00am - 5:00pm, Monday - Friday

Mental Health Case Managers coordinate services, design treatment plans and plan for crises to serve people of all ages with mental illness. Plans for mental health services are customized for each person's needs and designed to help people live with as much independence within the community as possible. Services may assist the person in gaining access to a variety of services including mental health, medical, social, educational, housing, vocational and recreational. MORE INFORMATION Case Management services connect individuals with supportive services to assist them to live independently within the community.
