DESCRIPTION: Offers adult mental health support services to those living with a serious mental illness, such as connection to resources and organized activities MORE INFORMATION Services may include: * Supportive therapy * Mental illness education * Independent living skills training * Case Management * Vocational support * Medication management * Housing assistance * Crisis assistance * Psycho-social rehabilitation Program services are primarily provided on an outreach basis with services typically occurring in the home or community. Outreach Centers offer persons the opportunity to be part of a social club, making friends and learning new skills. Application process

Referrals for these services must be made by a physician, therapist, or mental health case manager.

Fee

Accept many types of insurance, including Medical Assistance (MA) for covered services. At Human Development Center (HDC) no one is denied services due to inability to pay. Each location has a sliding fee scale available based on your income for people without insurance.
