Supportive Housing

DESCRIPTION: Housing for people with low income that includes coordination of other services for residents. The coordination professional can help residents identify resources, navigate systems or help with applying to additional programs. MORE INFORMATION Deaf Supported Living program provides culturally competent, integrated mental and medical health services to the Deaf, Deaf-Blind, and Hard-of-Hearing communities. Staff are both deaf and hearing individuals who are fluent in sign language. Services and on-site supervision are provided 24/7. Services include: * 24/7 emergency or crisis assistance * Access to Registered Nurses for medical support * Medical needs management * Medication management * Mental health symptom management * Living skills/foundational wellness development including: - Appointment scheduling, budgeting, and shopping, cooking and nutrition, employment skills, household management, using transportation, socialization and recreational skill development Uniform Disclosure of Assisted Living Services & Amenities (UDALSA) https://www.health.state.mn.us/facilities/regulation/assistedliving/udalsa/30393.pdf

Application process

Fee

Participants live in their own apartments and are responsible for their own food, clothing, and shelter expenses. Tenants pay rent for their unit. They work with each tenant and their financial worker to ensure that they receive financial assistance based on their eligibility. This may include Social Security Disability Insurance (SSDI), Minnesota Supplemental Aid (MSA), food stamps and medical assistance.

* 18+ who are Deaf, Hard of Hearing or Deaf-Blind * Mental health diagnosis * Community Access for Disability Inclusion (CADI) Waiver or Traumatic Brain Injury (TBI) Waiver eligible
