Serves people with mental illness, intellectual disabilities, dementia, brain injury, cognitive impairment, and chemical dependencies

Application process

Presbyterian Family Foundation has contracts with individual counties in order to take referrals to provide services for individuals for whom the county is responsible. Services are also available through private pay arrangements. Please call (320) 235-5897 or at (888) 560-5897 and ask to speak to Guardianship services.

Guardians and conservators are appointed by the court. They are chosen to make personal or financial decisions for someone who has been judged as unable to make these decisions for themselves, due to illness or other conditions. They are responsible to make some or all personal decisions for an individual and/or the management of their estate.
